Safe Medication Management in Hospital - Emphasis on Dispensing, and Administration

Abstract
Medication use is a complex process, and the possibility for risk is substantial. Although many errors result in no harm to the patient, the potential for serious injury with certain medication types, classes and routes of administration is substantial. In a study of closed claims, drug-related injuries were responsibility for the highest expenditures for claims related to procedures. Given the injuries and cost associated with preventable adverse drug events and the growing concern about medication errors by consumers, health care organizations have an obligation to invest time and effort to reduce and prevent them. Medication management is often an important component in the palliative, symptomatic and curative treatment of many disease and conditions. A safe medication management system addresses medication processes, including selection and procurement, storage, ordering and transcribing, preparing and dispensing, administration and monitoring. Effective and safe medication management involves multiple services and disciplines working closely together. This document was conducted to provide safe medication management focusing on medication errors. Medication error prevention approaches should be considered in the development of organizational systems and discuss methods of managing medication errors. The causes, types, recommendations for preventing, monitoring and management of medication errors were reviewed.
